CM is one of the most effective and evidence-based psychosocial interventions for substance use disorders. In over 100 randomized controlled trials and 9 meta-analyses, CM significantly and substantially enhances outcomes.
Despite CM’s proven success, about 90% of U.S. treatment programs don’t use it. Practical barriers to adoption include lack of funding and reimbursement, regulatory hurdles, and logistical challenges.

Addressing the U.S. Stimulant Crisis: A Whitepaper
The U.S. drug crisis has reached a critical juncture, with stimulant-related overdose deaths surpassing opioid-related fatalities for the first time in 2022. While the opioid epidemic has been addressed through expanded access to FDA-approved medications for Opioid Use Disorder (MOUD), no such pharmacological treatments exist for Stimulant Use Disorder (StUD). In response, the 2022 White House National Drug Control Strategy advocated for the adoption of evidence-based contingency management (CM) as a primary intervention for StUD. However, the Substance Abuse and Mental Health Services Administration (SAMHSA), the federal government's leading agency for public addiction treatment, has not fully embraced or funded this approach.This white paper examines the discrepancy between the established standard of care for StUD treatment and SAMHSA's implementation of motivational incentives through its State Opioid Response (SOR) grant program. By comparing two state programs - West Virginia's Medicaid CM project and New Jersey's SOR program - we explore the potential benefits of full-value CM versus the restricted incentives allowed under SAMHSA's guidelines. Both programs utilized the same digital health platform, DynamiCare Health, providing a unique opportunity to assess the feasibility of remote CM delivery and the behavioral impacts of different incentive amounts in real-world public health systems.
West Virgina Medicaid
West Virginia has seen a 490% increase in the rate of overdose deaths from methamphetamine in the past 5 years. With no FDA-approved medications for Stimulant Use Disorder (StUD), treatment options are limited. In partnering with DynamiCare Health, the aim of this pilot was to determine the feasibility & acceptability of deploying CM through DynamiCare’s platform. DynamiCare Health utilizes Contingency Management, “the only evidence-based treatment for stimulant use disorders” -Bill Haning MD, ASAM President-Elec.
After months of treatment, the results have been overwhelmingly positive. Out of 285 eligible patients, 91% were successfully contacted, and out of the enrollees, 65% completed the entirety of their 6-month program, and 87% of all tests taken by patients were negative for non-prescribed substances. In polling patients and providers for user satisfaction, the results were almost unanimous: the majority of patients and providers would recommend DynamiCare.
